Three Bottle System

The longer tube is under water-seal
Suction force is maintained as prescribed (5kpa)
No pulling or kinking of tubing
Chest drainage bottle is kept below chest level
All connections are properly connected and air-tight
The drainage system is patent and swinging
Any abnormality is reported promptly, e.g. chest drainage > 200/hr., disconnect of tubing, chest tube accidentally pulled/ slipped out, emphysema, empyema
Keep chest drain site dry and change dressing PRN
The amount and nature of drainage is recorded
Avoid clamp chest tubes from which air leakage is detected
Clamp and release chest drain and no suction force only for post-pneumonectomy patient
Ensure air vent open to air after suction is discontinued
Removal of chest tube if no air leakage or fluid output
